چکیده
Current Opinion in Critical Care 2009, 15:527–535 Purpose of review To bring together in one review article, the most current and relevant evidence relating to military trauma resuscitation. Recent findings The main themes highlighted by this review are coagulopathy of trauma shock (CoTS), damage control resuscitation, haemostatic resuscitation, the management of massive transfusion, use of adjuvant drugs for haemostasis and use of an empiric massive transfusion protocol. Summary The review aims to educate the readership in recent advances in trauma practice, culminating in a novel empiric massive transfusion algorithm seamlessly guiding the clinician through the initial resuscitation stage resulting in reduced mortality, morbidity, coagulopathy and decreased overall blood product usage.

With the development of British Army doctrine it is necessary to review the concepts underlying medical support to the modern battlefield. In particular, the provision of timely and balanced resuscitation to the high intensity battle is an issue requiring an understanding of both the tactical and the clinical demands. The wholesale application of civilian techniques and approaches to trauma man...

Hemorrhage remains a major cause of death on the battlefield in conventional warfare (Bellamy, 1984). Current dogma dictates that early, adequate fluid resuscitation is crucial to reduce the mortality and morbidity associated with hemorrhagic shock. Yet, despite much research in the field and years of resuscitating thousands of patients, the optimal fluid and resuscitation strategy for the trea...
متن کاملSmall-volume fluid resuscitation for the far-forward combat environment: current concepts.
Hemorrhage remains the primary cause of death on the battlefield in conventional warfare. With modern combat operations leading to the likelihood of significant time delays in air evacuation of casualties and long transport times, the immediate goals of the Army's Science and Technology Objectives in Resuscitation are to develop limited- or small-volume fluid resuscitation strategies, including...
متن کاملThe use of fresh whole blood transfusions by the SOF medic for hemostatic resuscitation in the austere environment.
The leading cause of death on the battlefield is uncontrolled hemorrhage. Non-compressible (truncal) hemorrhage is the cause over two thirds of these deaths. This makes truncal hemorrhage the leading cause of potentially survivable death on the battlefield. Over one third of the casualties who arrive at the emergency department (ED) or combat surgical hospital (CSH) in need of a blood transfusi...
